This form is for members of the Teachers’ Long-Term Disability Plan who want to maintain disability insurance coverage while on an approved leave of absence. This is not an application form to apply for disability benefits. If you want to apply for benefits, please contact a Teachers’ Long-Term Disability Plan case consultant and request an application package; the required forms are not available online.
